Capstone Project Focus

The proposal focuses on the following among the adult hypertensive population; better blood pressure management and medication compliance using a lifestyle change program led by nurses with a focus on dietary and physical exercise education.

PICOT Question Review

P: What is the association of subjecting adults with hypertension (P) to a nurse-led lifestyle modification program consisting of diet and exercise counselling (I) versus regular physician-based care (C) with blood pressure control and medication adherence (O) after 6 months (T)?

Assessment Plan Overview

Assessment 1 Finished project template and baseline proposal. Assessment 2: Formulate PICOT question and rationale. Assessment 3: Find one intervention (nurse-led lifestyle program). Assessment 4: Speak of implementation strategy. Assessment 5: design evaluation plan to evaluate success. Assessment 6: Put together papers and create dissemination plan.

Practicum Hour Expectations

At least 40 hours by Assessment 2, 80 by Assessment 3, etc. Hours are to be cumulative, and one sentence is to be written in every 2 hours recorded. Must explain certain actions (e.g., meeting with patients, discussing their treatment plans).

Turn in Excel record of hours with every assessment starting with Assessment 2. Make preceptor verify hours. Do not use such phrases as shadowed preceptor.

Preceptor and Site Engagement

Practicum activities will occur mostly on-site with the supervision of preceptors; not more than 20 percent of the hours can be remote (e.g., through Teams or Zoom). Find the time to visit leadership or clinical improvement meetings.
